Dear Parents/Guardians;

In previous years, paper copies of Athletic Acknowledgement of Risk and Informed Consent Forms were sent home with athletes and filled out prior to the start of each individual athletic season, which often meant that parents were filling out the same forms multiple times per year.

In an effort to reduce the number of forms our parents need to fill out and make this information more accessible to our coaches, we have created an online Athletic Acknowledgement of Risk and Informed Consent Form that will need to be filled out prior to your child trying out for any athletic team, and will cover all extracurricular athletic activities for your child as long as they attend their current school.
